末期腎不全で維持血液透析中の55歳男性が.左重症下肢虚血に対して繰り返し血管内治療を施行されていた.左足の感染の増悪で入院となり,抗生剤加療が開始された.左第3から5趾の重症下肢虚血に対して第7病日よりACE阻害薬を中止し第10病日より吸着式潰瘍治療法レオカーナ®を施行した.開始後6分で血圧低下を認めレオカーナ®中止と生理食塩水の補液を行ったがショック状態が持続した.ノルアドレナリン投与にてショック状態から離脱したが10時間後まで低血圧が持続した.レオカーナ®とACE阻害薬の併用はブラジキニンによるショック状態が出現する懸念があり禁忌である.メーカーから1か月前からの休薬期間が推奨されているものの明確な中止基準が存在しない.本症例ではレオカーナ®施行3日前よりACE阻害薬を中止としたがショック状態が出現し遷延した.レオカーナ®を施行する際のACE阻害薬の中止期間には特段の注意が必要であり報告する.
