Invasive pulmonary aspergillosis is increasingly reported in patients with influenza admitted to the intensive care unit (ICU). Classification of influenza-associated (IAPA) using current definitions for invasive fungal diseases has proven difficult, and our aim was develop case IAPA that can facilitate clinical studies. To investigate azole resistance in clinical Aspergillus isolates, we conducted prospective multicenter international surveillance. A total of 3,788 isolates were screened 22 centers from 19 countries. Azole-resistant A. fumigatus was more frequently found (3.2% prevalence) than previously acknowledged, causing resistant invasive and noninvasive aspergillosis severely compromising use azoles.

Oral triazole therapy is well established for the treatment of invasive (IPA), allergic (ABPA), and chronic pulmonary (CPA) aspergillosis, often long-term. Triazole resistance rates are rising internationally. Microbiological diagnosis aspergillosis limited by poor culture yield, leading to uncertainty about frequency resistance.
