Abstract Azoles are the mainstay of oral therapy for aspergillosis. Azole resistance in Aspergillus has been reported infrequently. The first resistant isolate was detected 1999 Manchester, UK. In a clinical collection 519 A. fumigatus isolates, frequency itraconazole 5%, significant increase since 2004 (p<0.001). Of 34 itraconazole-resistant isolates we studied, 65% (22) were cross-resistant to voriconazole and 74% (25) posaconazole. Thirteen 14 evaluable patients our study had prior azole...
To investigate azole resistance in clinical Aspergillus isolates, we conducted prospective multicenter international surveillance. A total of 3,788 isolates were screened 22 centers from 19 countries. Azole-resistant A. fumigatus was more frequently found (3.2% prevalence) than previously acknowledged, causing resistant invasive and noninvasive aspergillosis severely compromising use azoles.
Background The International Society for Human and Animal Mycology (ISHAM) working group proposed recommendations managing allergic bronchopulmonary aspergillosis (ABPA) a decade ago. There is need to update these due advances in diagnostics therapeutics. Methods An international expert was convened develop guidelines ABPA (caused by Aspergillus spp.) mycosis (ABPM; caused fungi other than adults children using modified Delphi method (two online rounds one in-person meeting). We defined...
Toll-like receptors (TLRs) are important components of innate immunity. We investigated the association between polymorphisms in TLR2, TLR4, and TLR9 genes susceptibility to noninvasive forms pulmonary aspergillosis. A significant was observed allele G on Asp299Gly (TLR4) chronic cavitary aspergillosis (odds ratio [OR], 3.46; P =.003). Susceptibility allergic bronchopulmonary associated with C T-1237C (TLR9) (OR, 2.49; =. 043). No particular polymorphism severe asthma fungal sensitization....
To describe temporal trends in the epidemiology, clinical management and outcome of candidemia intensive care unit (ICU) patients. This study was a retrospective analysis 1,392 episodes 647 adult ICU patients from 22 Brazilian hospitals. The characteristics these were compared two periods (2003-2007, period 1; 2008-2012, 2), predictors 30-day mortality assessed. The proportion who developed while increased 44 % 1 to 50.9 2 (p = 0.01). Prior exposure fluconazole before (22.3 vs. 11.6 %, p <...
In Brazil, human fungal infections are prevalent, however, these conditions not officially reportable diseases. To estimate the burden of serious diseases in 1 year based on available data and published literature. Historical official from were collected Brazilian Unified Health System Informatics Department (DATASUS). For for which no available, assumptions frequencies made by estimating The incidence (/1000) hospital admissions coccidioidomycosis was 7.12; histoplasmosis, 2.19;...
Corticosteroids have potent anti-inflammatory and immunosuppressive effects. Recently, these medications gained importance in the treatment of severe COVID-19. Here we present data demonstrating a marked (10-fold) increase frequency candidemia hospitalized patients with COVID-19 receiving corticosteroids Brazil. Overall mortality was 72.7%, despite antifungal therapy. Physicians should be aware potential risk for among severely ill high-doses corticosteroids.
Histoplasmosis is highly endemic in the American continent. This condition associated with a high mortality, particularly people living HIV/AIDS (PLWHA). Diagnosis of histoplasmosis usually late South America, as Histoplasma antigen detection rarely available. Here we determined prevalence, risk factors, and outcome PLWHA Brazilian hospitals. was prospective cohort study (2016–2018) involving 14 tertiary medical centers Brazil. We included hospitalized presenting fever additional clinical...
Invasive mycoses present a global challenge with expansion into new hosts, emergence of pathogens, and development multidrug resistance. In parallel, antifungal agents advanced laboratory diagnostic systems are being developed. response to these evolving challenges, the European Confederation Medical Mycology (ECMM) is committed providing international expertise, guidance, leadership key objectives improving diagnosis, treatment, outcome, survival persons invasive fungal diseases....
We present postmortem evidence of invasive pulmonary aspergillosis (IPA) in a patient with severe COVID-19. Autopsies COVID-19 confirmed cases were performed. The died despite antimicrobials, mechanical ventilation, and vasopressor support. Histopathology peripheral blood galactomannan antigen testing IPA. Aspergillus penicillioides infection was by nucleotide sequencing BLAST analysis. Further reports are needed to assess the occurrence frequency IPA SARS-CoV-2 infections, how they interact...
Despite the existence of endemic mycoses in Latin America and Caribbean, addition to a large population patients at risk for invasive mycoses, capability medical centres perform proper diagnosis mycology has not been studied region. Moreover, availability antifungal drugs region is unknown. Here, we report results survey involving 129 24 countries. Only 9% would have potential apply minimum standards mycology, as determined by European Confederation Medical Mycology. There an urgent need...
We sought to determine the prevalence of probable disseminated histoplasmosis among advanced HIV disease (AHD) patients in Nigeria. conducted a cross-sectional study 10 sites across 5 6 geopolitical zones identified with urinary samples containing CD4 cell counts <200 cells/mm3 or World Health Organization stage 3 4 who also had >2 clinical features histoplasmosis, and we tested them for Histoplasma antigen using enzyme immune assay. Of 988 participants recruited, 76 (7.7%) were...
Abstract Background Histoplasmosis is a major AIDS-defining illness in Latin America. Liposomal amphotericin B (L-AmB) the drug of choice for treatment, but access restricted due to high and hospitalization costs conventional long regimens. Methods Prospective randomized multicenter open-label trial 1- or 2-dose induction therapy with L-AmB versus control disseminated histoplasmosis AIDS, followed by oral itraconazole therapy. We subjects to: (i) single dose 10 mg/kg L-AmB; (ii) on D1, 5 D3;...
