42歳男性.膵体部腫瘤を指摘され紹介.CTでは膵体部に早期相で低吸収,遅延相で不明瞭な14mm大腫瘤を認めた.被膜や嚢胞を疑う像は認めなかった.FDG-PETでは膵体部腫瘤に一致する集積を認めた.確定診断と治療方針決定目的でEUS-FNAを施行した.1回目は断片化した腺房細胞を認めるのみであり,再度FNAを施行.SPNに典型的な偽乳頭状構造は認めなかったが,偏在性の核と淡好酸性の明るい細胞質を有し胞体の空胞変性が目立つ帰属不明瞭な均一な細胞からなる小集塊が散見された.SPNを疑い免疫染色を行った.β-catenin,CD10陽性所見が得られ,FNA検体でSPNと術前診断した.膵体尾部切除術を行い,切除病理組織においてSPNと最終診断した.典型的な偽乳頭状構造を認めなくても,細胞質空胞が目立つ帰属不明瞭な細胞がみられた場合はSPNを疑い,免疫染色を含めたさらなる検討が必要であると考えられた.
