膵仮性嚢胞を合併した自己免疫性膵炎(AIP)12症例をもとに,膵仮性嚢胞の形成機序とステロイドの有効性について検討した.膵仮性嚢胞の発生部位は全例膵体尾部であり,10例の83%に膵管狭細化に伴う閉塞性膵炎を認めた.膵酵素上昇(75%)および腹痛(50%)も高率に認めた.ステロイド治療により膵仮性嚢胞は全例で消失し,膵管狭細像および閉塞性膵炎像も改善した.さらに,膵酵素上昇および腹痛も全例で改善した.以上より,AIPに合併する膵仮性嚢胞の形成機序は閉塞性膵炎に起因するものであり,AIPに合併する膵仮性嚢胞には閉塞性膵炎の解除を目的としたステロイド投与は有効であった.AIPの疾患概念が発信された当初は,閉塞性膵炎および膵仮性嚢胞の合併は稀と考えられていたが,今回の検討から膵仮性嚢胞の診療においてはAIPの可能性も念頭に置いた的確な診断と治療法の選択が重要と考えられた.
膵神経内分泌腫瘍は比較的稀な疾患であるが,近年注目されている領域である.機能性腫瘍と非機能性腫瘍に分けられるが,症状が認められてから診断までに時間がかかった症例も少なくなく,診断時既に遠隔転移が認められる症例も多い.基本的に悪性腫瘍であるが,治療方針を決定する上で,正確な診断(存在診断・局在診断・病理診断)が極めて重要である.多発性内分泌腫瘍症1型など遺伝性疾患を合併することがあり,治療開始前に鑑別しておく必要がある.診断における血中クロモグラニンA測定やソマトスタチンレセプターシンチグラフィによる腫瘍の局在診断は有用であり,欧米では使用されているが本邦では保険収載されていない.国際標準的な診断体系の本邦における確立が今後の課題である.
膵神経内分泌腫瘍(pancreatic neuroendocrine neoplasm:PanNEN)は希少疾患であるが,近年増加傾向である.超高齢社会に伴い今後はPanNEN患者の高齢化も予想される.PanNENの唯一の根治治療は外科切除であり,適応を選んで外科切除を施行した場合,高齢者においても安全な外科切除が可能である.一方で,年齢や併存疾患,腫瘍径などから,経過観察の選択肢も考慮する.高分化型の膵神経内分泌腫瘍(pancreatic tumor:PanNET)に対する全身療法として,ソマトスタチンアナログ,分子標的薬,細胞障害性抗がん剤,放射性核種標識ペプチド治療,などが挙げられる.各種治療法の特徴・有害事象を理解した上で,高齢者PanNETに適用することになるが,外科手術と同様に,併存疾患や認知機能などの事前評価が重要となる.
