小児気管支喘息患者に対してダニアレルゲン特異的免疫療法(皮下免疫療法,舌下免疫療法)とプラセボを比較した試験は16編あり,その中で皮下免疫療法では喘息症状,頓用薬の使用,全身性ステロイド投与,長期管理薬の使用量,呼吸機能において,舌下免疫療法では喘息症状,呼吸機能においてダニアレルゲン特異的免疫療法が効果的であることが示された.しかしエビデンスレベルが総じて低く,日本人小児を対象とした試験がなかった.また本邦では皮下免疫療法は喘息への保険適応があるものの,舌下免疫療法は現時点でアレルギー性鼻炎のみが適応となっており,その実施には喘息が十分にコントロールされていることが注意点として挙げられている.アレルゲン特異的免疫療法には副反応も少なからず報告されており,治療効果と副反応のバランスを考慮していく必要がある.
