Extramedullary plasmacytomas are rare tumors. To the best of our knowledge, only very few cases have been previously reported on positron emission tomography (PET) with glucose analog 2-deoxy-2-[F-18] fluoro-D-glucose (F-18 FDG), which showed markedly increased F-18 FDG accumulation in primary lesions. We present a new case PET/CT images showing low uptake nasopharynx plasmacytoma. Clinicians should be aware this phenomenon and potential for false negatives PET/CT.
