副乳癌の1例を経験したので若干の文献的考察を加えて報告する.【症例】88歳,女性.平成14年3月,右腋窩の腫瘤性病変を自覚し当科受診.右腋窩に直径2cm大の腫瘤を認め, 3/25局所麻酔下に生検施行.術後の病理学的検索にて副乳癌の診断を得た. 4/19全身麻酔下に遺残腫瘤切除・腋窩リンパ節郭清(level I)を施行.切除標本に明らかな癌の遺残,腋窩リンパ節転移を認めなかった.現在外来にてAnastrozole内服下に経過観察中である.【考察】副乳癌は稀な疾患で, 2003年までに100例の報告がある.腋窩腫瘤の診断では副乳癌を念頭に置いて検索するべきであると考えられた.また,手術においては乳房切除の必要性を検討するため,術前に腫瘤全体の組織学的検索をするべきであると考えられた.

35歳男性．原因不明の心停止蘇生後の経口気管挿管中に，リユースタイプバイトブロックの金属内筒が脱落し気管支内に迷入した．リユースタイプバイトブロックは，金属内筒をゴムで被覆した構造が多いが，度重なる咬合や滅菌によりゴムの劣化が進み，ゴム部分が破損した場合に内部の金属内筒が脱落する可能性を有している．本症例における金属内筒の脱落した経緯を報告するとともに，その発生原因と対応策について考察した．この症例を契機に，当院ではディスポーザブルタイプバイトブロックの使用に変更した．
