Background: Research on the optimal duration of immunosuppressive therapy (IST) and outcome upon its discontinuation in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) remains limited. Objective: To evaluate outcomes following IST MOGAD. Methods: This multicenter retrospective study collected data from 333 MOGAD patients Korea. Among 273 who received IST, 41 discontinued were analyzed. Results: The median age at onset was 38.3 years (interquartile range (IQR),...
Association of human leukocyte antigen (HLA) with anti-aquaporin-4 immunoglobulin G-positive neuromyelitis optica spectrum disorder (AQP4-IgG NMOSD) has been reported. However, this association in the Korean population not previously investigated. We aimed to evaluate whether specific HLA subtypes were associated patients AQP4-IgG NMOSD and genotype is clinical features. compared 122 those 485 (HLA-A, HLA-B, HLA-C, HLA-DRB1, HLA-DQB1) 173 (HLA-DPB1) healthy controls. In addition, we features...
Treatments for neuromyelitis optica spectrum disorder (NMOSD) such as eculizumab, ravulizumab, satralizumab, and inebilizumab have significantly advanced relapse prevention, but they remain expensive. Rituximab is an off-label yet popular alternative that offers a cost-effective solution, its real-world efficacy needs better quantification guiding the application of newer approved NMOSD treatments (ANTs). This study aimed to determine rituximab failure rates anticipate demand ANTs aid in...
Abstract Background This multicenter clinical trial evaluated the stand-alone efficacy and improvements in diagnostic accuracy of early-career physicians using a deep learning-based software to detect large vessel occlusion (LVO) CT angiography (CTA). Methods pivotal included 595 ischemic stroke patients from January 2018 September 2023. Standard reference LVO locations (intracranial internal carotid artery [ICA], M1, or M2) were determined by consensus among three expert vascular...
ABSTRACT This study assesses the outcomes of immunotherapy discontinuation in patients with relapsing seronegative idiopathic myelitis (SIM), a condition that remains uninvestigated due to its rarity. We reviewed records from 77 SIM at National Cancer Center Korea, focusing on 11 who discontinued treatment after median relapse‐free period over 5 years. Notably, all remained for 36.5 months following discontinuation. Our findings suggest can be feasible therapeutic strategy selected have...
Alexander disease (AxD) is a rare autosomal dominant astrogliopathy caused by mutations in the gene encoding for glial fibrillary acidic protein. AxD divided into two clinical subtypes: type I and II AxD. Type usually manifests bulbospinal symptoms occurs second decade of life or later, its radiologic features include tadpole-like appearance brainstem, ventricular garlands, pial signal changes along brainstem. Recently, eye-spot signs anterior medulla oblongata (MO) have been reported...
Background To evaluate the stand-alone efficacy and improvements in diagnostic accuracy of early-career physicians artificial intelligence (AI) software to detect large vessel occlusion (LVO) CT angiography (CTA). Methods This multicenter study included 595 ischemic stroke patients from January 2021 September 2023. Standard references LVO locations were determined by consensus among three experts. The AI was benchmarked against standard references, its impact on four residents involved care...
Abstract Background Accurate infarct volume measurement requires manual segmentation in diffusion weighted image (DWI) which is time-consuming and prone to variability. We compared two DWI programs based on deep learning the apparent coefficient threshold (JBS-01K RAPID DWI, respectively) a comprehensive stroke center. Method included 414 patients whose were evaluated using JBS-01K. used Bland-Altman plot compare estimated manually segmented volumes. R-squared, root mean squared error,...
Anti-melanoma differentiation-associated gene 5 (anti-MDA5) antibody is a myositis-specific marker detected in clinically amyopathic dermatomyositis (DM). DM with anti-MDA5 can be accompanied by rapidly progressive interstitial lung disease (RP-ILD) and other autoimmune disorders. Until now, only one case of neuromyelitis optica (NMO) anti-MDA5-positive has been reported worldwide, which the patient achieved favorable outcome intensive immunotherapy. We report NMO complicated ILD rheumatoid...
To investigate the clinical, laboratory, and radiological features of meningitis after lumbar epidural steroid injection (M-ESI) without accompanying spinal infection, data patients with admitted between January 2014 December 2021 in a single center were retrospectively reviewed. Among them, recent history ESI identified, their medical records collected. Patients concomitant infections other than meningitis, including abscess, excluded. Seven M-ESI identified. All presented headache fever...
Neurofibromatosis type 1 (NF-1) can manifest with various neurological symptoms. However, sensory ataxia has not been reported.A 44-year-old man NF-1 presented several weeks of unsteady gait. He was diagnosed gastric neuroendocrine tumor multiple hepatic metastases 6 years ago and received palliative chemotherapy. Neurological examination revealed veering to the right side no motor weakness.Clinical manifestations electrodiagnostic studies suggested dysfunction thoracic dorsal column (DC)....
Abstract This study aimed to elucidate whether low-density lipoprotein cholesterol (LDL-C) levels differentially affect functional outcomes after intravenous thrombolysis (IVT) depending on prestroke statin use. Patients with acute ischemic stroke treated IVT were categorized into low, intermediate, and high LDL-C groups based at admission (< 100/100–130/ > 130 mg/dl, respectively). Multivariable logistic regression analyses performed explore the relationships between clinical (good 3...
To analyze the clinical phenotype of hereditary spastic paraplegia (HSP) caused by SPG11 mutations (SPG11-HSP).Among 17 patients with sporadic HSP who performed whole exome sequencing analysis, six were diagnosed SPG11-HSP. The and radiologic findings results electrodiagnostic neuropsychologic tests reviewed retrospectively. The median age at onset was 16.5 years (range, 13-38 years). Progressive paraparesis a core feature, rating scale score 24/52 16-31 points). Additional major symptoms...
Posterior reversible encephalopathy syndrome (PRES) is a neurological disorder characterized by its reversibility both radiologically and clinically, it typically has favorable prognosis. Its etiopathogenesis not clear, although known that involves endothelial dysfunction of the posterior cerebral vasculature, leading to failed autoregulation. We report patient with PRES may have been caused an immune-mediated response decitabine. Key words: encephalopathy; Decitabine.
