Tendon xanthoma is very rare. We report FDG PET/CT findings of tendon in a 33-year-old man. On image, it manifested as multiple symmetric nodules with an SUVmax 9.37, located the tendons both elbows, knees and ankles. The final pathology supported diagnosis xanthoma.

Abstract Langerhans cell histiocytosis only involving the posterior elements of spine is rare.
