症例は47歳女性.2年前に右乳癌に対して切除術を受け,その後,タモキシフェンを内服していた.タモキシフェン内服開始27ヶ月後に左側腹部痛が出現し,血液検査で膵酵素が上昇し,CTでは,膵腫大と左前腎傍腔に脂肪壊死を認めたため,急性膵炎と診断された.絶食と補液などの保存的治療で軽快し,入院13日目に退院した.入院時,血中トリグリセリド値が5512mg/dlと著明に上昇しており,急性膵炎の原因としてタモキシフェン投与による高脂血症が考えられた.入院時からタモキシフェンを中止したところ,血中トリグリセリド値は速やかに低下し,退院4ヶ月後には基準値内となった.タモキシフェンによる著明な高脂血症とそれによる急性膵炎発症の頻度は稀だが,重篤になることがある.タモキシフェン投与中の患者は,血中トリグリセリド値の定期的な経過観察が必要であり,上昇していれば,迅速な対応が必要である.
