Objective:
症例は48歳, 男性. 糖尿病性腎症による末期腎不全で, 平成2X年4月上旬に血液透析 (HD) を導入された. 導入直後より39°C台の発熱を認めたが原因不明で, 抗生物質による改善より細菌性感染と考えた. 以後, 近医でHDを施行していたが, 8月下旬に嘔吐があり腹部腫瘤を指摘され, 再度精査加療の目的で入院となった. 画像検査上, 腹腔内小腸周囲に12×5×8cmの被膜を有する囊胞性腫瘤を認めた. 手術所見は腹膜の癒着と臓側・壁側腹膜に充実性腫瘤を多数認め, 腫瘤自体は癒着が強く切除不能であった. 進行性の悪性軟部腫瘍と考えられたが, 他臓器の病変は認めなかった. 病理組織所見では, 高度の好中球・マクロファージの浸潤, 巨細胞状の多核細胞を散見し核の多形性を認めた. 免疫染色では, 特異的マーカーは陰性であった. 以上より, 炎症性悪性線維性組織球腫と診断された. 透析症例における本疾患については既報がなく, 貴重な症例と考え報告する.
目的: 血管中膜石灰化は慢性腎臓病 (chronic kidney disease;CKD) 患者に特徴的な合併症であり, 心血管系イベントや死亡率を増加させる原因となっている. 症例は54歳,女性.平成3年,IgA腎症による慢性腎不全のため腹膜透析(PD)を施行し,平成12年から血液透析(HD)を併用した.平成19年6月,PDカテーテルを抜去した.その直後から腹水の貯留を認めるようになり,被嚢性腹膜硬化症(EPS)を疑い副腎皮質ステロイド内服治療を行ったが,腹水は改善しなかった.平成20年2月,腹痛と血圧低下で緊急入院し,腹水穿刺でゼリー状の貯留液を認めた.直ちに腹部MRIを行ったところ,左卵巣腫瘍の腫大と腹腔内に多量の腹水を認め,卵巣腫瘍と腹膜炎を疑い緊急手術となった.開腹所見では,左卵巣の腫大と多数の大腸憩室,腹腔に充満したゼリー状内容物を認め,回盲部周囲の内容物は混濁していた.病理組織所見から左卵巣の粘液性嚢胞性腫瘍の穿破による腹膜偽粘液腫と憩室炎による腹膜炎と診断された.PD中止後の腹水貯留に対して,体液過剰やEPSのみならず,稀ではあるが粘液や漿液産生腫瘍などの発症も考慮し,病態の検索が必要であると考えられた.
【目的】血液透析患者の低亜鉛血症に伴う貧血に対する亜鉛補充療法の投与基準がいまだ確立されていないことから, 本療法の有用性と投与方法について検討した. 【対象・方法】エリスロポエチン9,000U/週以上の投与で血清亜鉛値79μg/dL以下の血液透析患者 (33名) に亜鉛45~50mg/日を3か月間補充投与した. 【結果】血清亜鉛値は, 開始時57.9±9.71μg/dLであったが, 3か月後は92.4±24.2μg/dLと有意に改善した (p<0.0001). しかし, 血清銅値の低下が23名に認められた. 赤血球造血刺激因子製剤低反応性指数は, 開始時23.1±15.9U/week/kg/Hb (g/dL) であったが, 3か月後は12.4±6.1U/week/kg/Hb と有意な改善がみられた (p=0.0002). 【結語】血液透析患者における貧血の管理に亜鉛補充療法は有用である. 本療法は短期間であっても低銅血症のリスクが高く注意が必要である.
