症例は52歳の男性で, RaRb直腸癌に対して低位前方切除術を施行した. 総合所見ではA, N1, H0, P0, M0, Stage IIIaであった. 術後15病日, 発熱, 混濁尿を認めた. 尿路感染の診断で, 保存的加療を施行し, 一旦は病状改善した. その後, 下血, 血尿, 気尿を認め, CT, 注腸造影X線検査で縫合不全, 直腸精嚢瘻と診断した. 後日, 横行結腸人工肛門造設術を施行した. 人工肛門造設術後7か月で縫合不全, 直腸精嚢瘻の消失を確認し, 人工肛門を閉鎖した.本邦では直腸癌術後合併症として, 直腸精嚢瘻の報告はなく, 文献的考察を加え報告する.
