胆管気管支瘻は胆汁が気管支に流出する稀な疾患であり,化学性肺炎を引き起こし治療に難渋する場合が多く,治療法は確立されていない。今回我々が経験した症例は82歳,男性。 肝細胞癌治療後に胆管気管支瘻を発症し,慢性気道感染を繰り返していた。横隔膜ヘルニアを併発しており,手術侵襲や合併症のリスクを考慮して外科的根治術は困難であると判断した。姑息的に胆管外瘻チューブを留置していたが,チューブ閉塞に伴い胆汁流出による化学性肺炎を発症した。呼吸状態が悪化したためICUに入室し,健側肺保護のためにダブルルーメンチューブを用いて分離肺換気で管理した。胆管外瘻チューブを交換後,気管支鏡で胆汁流出の程度を評価しながら人工呼吸器設定や体位交換,経腸栄養開始時期などを検討することで肺炎増悪を防ぎ良好な経過を辿った。胆管気管支瘻による化学性肺炎では,外瘻を確立させて速やかに胆汁流出量を減少させ,分離肺換気で健側肺を保護することが重要である。
