TAFRO症候群は,血小板減少(thrombocytopenia),腔水症(anasarca),発熱(fever),骨髄線維症(myelofibrosis),腎機能障害(renal dysfunction),臓器腫大(organomegaly)を特徴とする.症例は66歳,男性.7カ月前に血小板数減少を指摘され,約1カ月前より腹痛,胸腹水貯留が生じた.前医入院後に腎機能障害が出現し,当院転院となり,TAFRO症候群と診断した.比較的急峻な経過を辿るもステロイドおよびシクロスポリンによる治療で改善した.血小板減少を伴う腎機能障害の鑑別にTAFRO症候群を挙げることが肝心である.

非閉塞性腸管虚血症 (non-occlusive mesenteric ischemia : NOMI) は, 腸間膜動静脈に閉塞や血栓を認めない腸管虚血症である. 血液透析 (HD) 患者におけるNOMI発症率はまれだが, 非透析患者に比べて100倍程度増加し, かつ, 致死率が極めて高く, 迅速な診断と加療を要する. 血圧低下を契機にNOMIを発症したHD2症例を経験した. 症例1はHD歴2か月の52歳男性で低左心機能に伴う低血圧を契機に発症, 症例2はHD歴5年の88歳女性で出血に伴う低血圧を契機に発症した. NOMI発症誘因として全身の低灌流状態に伴う血管攣縮が示唆されている. またHD患者のprofileとNOMIのrisk factorは酷似している. NOMIに特異的症状や検査所見はなく, HD中の血圧低下に腹痛を併発した場合にNOMIを念頭におくことが重要であり, 簡便に施行できるdynamic造影CT検査が診断に有用である. NOMIは予後不良であり, 発症予防として, 透析間体重増加の管理などの患者教育およびHD中の血圧管理が重要である.
