Surgical occlusion of the left atrial appendage has been hypothesized to prevent ischemic stroke in patients with fibrillation, but this not proved. The procedure can be performed during cardiac surgery undertaken for other reasons.
Background: Atrial fibrillation (AF) is a common complication after coronary artery bypass grafting operation (CABG). Experimental data have shown antiarrhythmic effects of n-3 polyunsaturated fatty acids (PUFA) on myocardial cells. Orally administered PUFA could significantly reduce the rate postoperative AF. We assessed efficacy for prevention AF CABG. were given intravenously to prevent variation in bioavailability.
The antiplatelet treatment strategy providing optimal balance between thrombotic and bleeding risks in patients undergoing coronary artery bypass grafting (CABG) is unclear. We prospectively compared the efficacy of ticagrelor aspirin after CABG.We randomly assigned double-blind fashion scheduled for CABG to either 90 mg twice daily or 100 (1:1) once daily. primary outcome was composite cardiovascular death, myocardial infarction (MI), repeat revascularization, stroke 12 months CABG. main...
We used individual patient data from 4 of the largest contemporary coronary bypass surgery trials to evaluate differences in long-term outcomes when radial artery (RA), right internal thoracic (RITA) or saphenous vein graft (SVG) are complement left artery-to-left anterior descending graft.Primary outcome was all-cause mortality. Secondary a composite major adverse cardiac and cerebrovascular events (all-cause mortality, myocardial infarction stroke). Propensity score matching Cox regression...
Objective: Review of the results surgical correction atrioventricular septal defects (AVSD), identification risk factors for mortality and failure left AV valve repair determination impact cleft closure on postoperative function. Methods: Between 1975 1995, 121 consecutive patients (55 males, 66 females) underwent surgery biventricular AVSD with a median age 1.2 years weight 7.6 kg. Sixty-five had complete AVSD, 17 an intermediate type, 39 partial AVSD. The (MV) was closed in 53 (43.8%)....
This study identifies high-risk octogenarians for surgical aortic valve replacement (AVR) because with the current advances in transcatheter therapy, a definition of patient selection criteria is essential.Between 1996 and 2006, 493 consecutive symptomatic stenosis underwent AVR without (51%) concomitant coronary artery bypass grafting (CABG). To identify groups, risk factors 6-month mortality were determined using multivariable logistic regression. The 30-day rate was 8.4% it increased up to...
Background Remote ischemic preconditioning ( RIPC ) has been suggested to protect against certain forms of organ injury after cardiac surgery. Previously, we reported the main results RIPHeart (Remote Ischemic Preconditioning for Heart Surgery) Study, a multicenter trial randomizing 1403 surgery patients receiving either or sham‐ . Methods and Results In this follow‐up paper, present 1‐year composite primary end point its individual components (all‐cause mortality, myocardial infarction,...
Following cardiac surgery, postoperative cognitive decline (POCD) is a common complication that can impair the quality of life and increase mortality. The aim this study was to investigate whether early training decrease POCD after surgery. The multi-centred, two-arm, randomized (1:1 ratio), controlled trial involving older patients undergoing elective heart valve surgery with extracorporeal circulation. Recruitment took place at Department Cardiac Surgery Kerckhoff-Clinic in Bad Nauheim...
Objective Stroke and transient ischemic attacks are considered as clinical manifestations of atherosclerotic disease due to on-going vascular inflammation finally atherothrombosis the carotid arteries. MicroRNAs (miRNA/miR) known be involved in plaque destabilization. The aim this study was analyze expression profile selected miRNAs endarterectomy specimen from arteries that were taken patients with asymptomatic symptomatic plaques. Methods Results 11 their analyzed using real-time RT-PCR....
Objective: After transluminal endovascular implantation of a new valved stent, the aim this study was to evaluate feasibility using high-pressure water stream endovascularly resect human calcified aortic valves. Methods: First, valves were excised and then resected in vitro determine optimal jet parameters. Second, healthy porcine ablated possible destruction surrounding anatomy. Third, resection performed endoluminally by introducing microsystemic tools into descending aorta, passing them...
The aim of our study was the identification genetic variants associated with postoperative complications after cardiac surgery. We conducted a prospective, double-blind, multicenter, randomized trial (RIPHeart). performed genome-wide association (GWAS) in 1170 patients both genders (871 males, 299 females) from RIPHeart-Study cohort. Patients undergoing non-emergent surgery were included. Primary endpoint comprises binary composite complication rate covering atrial fibrillation, delirium,...
Abstract Background Heart surgery is a risk factor for objectively and subjectively assessable postoperative cognitive decline (POCD), which relevant everyday life. The aim of this study was to investigate whether early training has an impact on health‐related quality life failures in daily living after cardiac surgery. Methods two‐arm, randomized, controlled, outcome‐blinded trial involving older patients undergoing elective heart valve with extracorporeal circulation (ECC). Recruitment...
Previous studies in adult patients and animal models have demonstrated that pulse pressure variation (PPV) stroke volume (SVV) can be used to predict the response fluid administration. Currently, little information is available on performance of these variables infants neonates. The aim our study was assess whether PPV SVV responsiveness an model investigate influence different tidal volumes applied.PPV were monitored by contour analysis 19 anaesthetized paralysed piglets during ventilation...
Although intra-aortic balloon pumping (IABP) is the most frequently used mechanical cardiac assist device in cardiothoracic surgery, there are only guidelines for substantive sections of aortic counterpulsation including prophylactic and postoperative use. In contrast, evidence-based recommendations still lacking concerning intraoperative use, management, contraindication other relevant issues. According to international surveys, important aspects IABP usage show a wide variation clinical...
