今回われわれは腹部造影CT検査にて術前診断し,腹腔鏡下に切除しえた大網捻転症の1例を経験したので報告する.症例は45歳女性で,5日前からの右下腹部痛を主訴に当院を受診した.来院時,右下腹部に手拳大の硬い腫瘤を触知し,同部位に著明な圧痛を認めた.腹部造影CT検査にて大網捻転症と診断し,同日,緊急腹腔鏡下手術を施行した.手術所見では,血性腹水を中等量認め,右下腹部の大網が反時計方向に4回転捻転し,末梢側の大網は壊死に陥っていた.大網壊死部分を切除した.術後経過は良好で,術後6日目に退院した.大網捻転症は比較的まれな疾患であり,術前診断は一般に困難であるとされている.本症例では,腹部造影CT検査にて術前診断が可能であり,腹腔鏡下手術が治療に有用であった.
