今回われわれは腹部造影CT検査にて術前診断し，腹腔鏡下に切除しえた大網捻転症の1例を経験したので報告する．症例は45歳女性で，5日前からの右下腹部痛を主訴に当院を受診した．来院時，右下腹部に手拳大の硬い腫瘤を触知し，同部位に著明な圧痛を認めた．腹部造影CT検査にて大網捻転症と診断し，同日，緊急腹腔鏡下手術を施行した．手術所見では，血性腹水を中等量認め，右下腹部の大網が反時計方向に4回転捻転し，末梢側の大網は壊死に陥っていた．大網壊死部分を切除した．術後経過は良好で，術後6日目に退院した．大網捻転症は比較的まれな疾患であり，術前診断は一般に困難であるとされている．本症例では，腹部造影CT検査にて術前診断が可能であり，腹腔鏡下手術が治療に有用であった．
