背景.ドライバー遺伝子変異/転座陰性のIV期非小細胞肺癌における免疫チェックポイント阻害薬(immune checkpoint inhibitor:ICI)の有効性が知られている一方で様々な免疫関連有害事象(immune-related adverse events:irAEs)が報告されており,その一つに硬化性胆管炎が存在する.しかし報告は少なく,その臨床経過や治療については明らかではない.症例.当院で硬化性胆管炎を発症した3例の臨床経過について検討した.3例はICI投与中ないし投与後に胆道系酵素優位の肝機能障害を認めた.Magnetic resonance cholangiopancreatography(MRCP)で肝内胆管に病変が限局していた1例はステロイドにより改善したが,肝内胆管及び肝外胆管に病変を認めた2例はステロイド反応性が良好と不良に分かれた.結論.irAE硬化性胆管炎のステロイド反応性の予測因子について,当院の3例の臨床経過に論文的考察を加えて検討した.肝内胆管に病変が限局する場合はステロイド反応性が良好である可能性が示唆された.
