Immune-checkpoint-inhibitor (ICI)-associated myotoxicity involves the heart (myocarditis) and skeletal muscles (myositis), which frequently occur concurrently are highly fatal. We report results of a strategy that included identification individuals with severe ICI myocarditis by also screening for managing concomitant respiratory muscle involvement mechanical ventilation, as well treatment CTLA4 fusion protein abatacept JAK inhibitor ruxolitinib. Forty cases definite were pathologic...

Rejection is one of the major causes late cardiac allograft failure and at present can only be diagnosed by invasive endomyocardial biopsies. We sought to determine whether microRNA profiling could serve as a non-invasive biomarker rejection. We included 113 heart transplant recipients from four referral French institutions (test cohort, n = 60, validation 53). In test we compared patients with acute biopsy-proven rejection (n 30) matched control without 30), assessing microRNAs expression in...

Antibody-mediated rejection (AMR) contributes to heart allograft loss. However, an important knowledge gap remains in terms of the pathophysiology AMR and how detection immune activity, injury degree, stage could be improved by intragraft gene expression profiling. We prospectively monitored 617 transplant recipients referred from 4 French centers (January 1, 2006-January 2011) for AMR. We compared patients with (n=55) a matched control group 55 without characterized all using histopathology...

Expanded clinical experience with patients taking antiangiogenic compounds has come increasing recognition of the renal adverse effects. Because histology is rarely sought in those patients, consequences are underestimated. Antiangiogenic-treated-cancer who had a biopsy for effects from 2006 to 2013, were included current study. Clinical features and histologic findings reviewed. Our cohort was 100 (58 women) biopsy-proven kidney disease using anti-vascular endothelial growth factor (VEGF)...

In heart transplantation, there is a lack of robust evidence the specific causes late allograft failure. We hypothesized that substantial fraction failing allografts may be associated with antibody-mediated injury and immune-mediated coronary arteriosclerosis. included all patients undergoing retransplantation for terminal failure in three referral centers. performed an integrative strategy phenotyping by assessing vascular tree including histopathology immunohistochemistry together...

Noninfectious mixed cryoglobulinemic GN (MCGN) has been poorly investigated. We analyzed presentation and outcome of 80 patients with biopsy-proven MCGN, which were identified in the retrospective French CryoVas survey. MCGN was related to primary Sjögren's syndrome 22.5% lymphoproliferative disorders 28.7% patients, defined as essential 48.8% patients. At presentation, hematuria, proteinuria ≥1 g/d, hypertension, renal failure observed 97.4%, 84.8%, 85.3%, 82.3% cases, respectively....

Abstract Background Fulminant myocarditis (FM) is a severe condition primarily triggered by viruses. Anti-RNA polymerase III autoantibodies (RNApol3) which are typically found in patients with systemic sclerosis, have been reported influenza-related FM. Our objective to provide additional insight into RNApol3-associated Methods We retrospectively included all admitted our institution between January 2013 and June 2023 acute positive serum RNApol3. compared their characteristics, etiologies,...

Data regarding the incidence and outcome of renal involvement in patients with inflammatory myopathies (IM) remain scarce. We assessed causes acute kidney injury (AKI) chronic disease (CKD) 150 dermatomyositis, polymyositis, antisynthetase syndrome followed 3 French referral centers. Renal occurred 35 (23.3%) patients: AKI 16 (10.7%), CKD 31 (20.7%) patients. The main cause was drug or myoglobinuria-induced tubular necrosis. Objective. To report the incidence, predictors, and outcome of rituximab (RTX)-associated autoimmune disease flare. Methods. We conducted a retrospective study in tertiary referral center from 2005 to 2015. Disease flare was defined as onset new organ involvement or worsening within 4 weeks following RTX. Results. Among 185 patients, we identified 7 flares (3.4%). All were due type II mixed cryoglobulinemia vasculitis. Vasculitis occurred after median time 8 days (range 2–16) RTX infusion...
