免疫抑制薬内服患者への生ワクチン接種は添付文書上では禁忌とされている。しかしながら,免疫抑制薬内服患者はウイルス感染症で致命的となるリスクが高く,接種が望ましいという意見もあり,予防接種ガイドラインでは禁忌にはなっていない。今回われわれは,免疫抑制薬内服中で,麻疹・風疹・水痘・おたふくかぜについて酵素抗体法で (-) か (±) を示した腎疾患または膠原病患者に,当院倫理委員会承認後に患者毎に十分な説明を行い,生ワクチン接種を行い,抗体獲得率と有害事象について前向きに検討した。40名 (1~24歳) に,55接種 (MRワクチン22接種,水痘ワクチン18接種,おたふくかぜワクチン15接種) 施行した。抗体獲得率は,麻疹 (90%) と風疹 (93%) は高く,水痘 (44%) およびおたふくかぜ (43%) は低かった。2名に発熱,1名に発疹,1名にネフローゼの再発を認めたが,重篤な有害事象はなかった。免疫抑制薬内服中でも生ワクチン接種が有効である可能性が示唆された。
