目的 通过对17例子宫瘢痕处妊娠(CSP)临床资料的分析,探讨其主要病因及有效的治疗方法。 方法 对17例CSP患者的临床资料进行回顾性分析。 结果 17例CSP患者平均年龄(32.6±6.6)岁;孕次≥3次15例;剖宫产次数≥2次8例;术前平均血HCG(54 093.8±42 509.4)mIU/mL;Ⅰ型CSP 14例,Ⅱ型CSP 3例;12例行宫腹腔镜联合监测下吸宫术,3例行宫腔镜下吸宫术,1例行B超监测下吸宫术,1例行经腹子宫瘢痕痕处妊娠楔形切除术,手术均顺利,均保留子宫,保留了生育功能;平均手术时间(68.4±35.2)min,术中平均出血量(70.6±93.6)mL;术后1~3周随访血HCG基本降至正常,达到治愈标准。 结论 CSP的发生与孕次及剖宫产次数关系密切,进行术前预处理(即术前采取米非司酮口服及单剂量一次使用MTX杀胚治疗,术前2 h米索前列醇400 μg纳阴)后行宫腹腔镜联合监测下吸宫术,对于子宫瘢痕处妊娠是一种安全、快捷、有效的治疗方法,值得临床推广和应用。
