Risk factors for coronavirus disease 2019 (COVID-19) death in sub-Saharan Africa and the effects of human immunodeficiency virus (HIV) tuberculosis on COVID-19 outcomes are unknown. We conducted a population cohort study using linked data from adults attending public-sector health facilities Western Cape, South Africa. We used Cox proportional hazards models, adjusted age, sex, location, comorbidities, to examine associations between HIV, tuberculosis, 1 March 9 June 2020 among (1) "active...
Introduction New biomarkers are urgently needed to detect pancreatic ductal adenocarcinoma (PDAC) at an earlier stage for individualized treatment strategies and improve outcomes. Autoantibodies (AAbs) in principle make attractive as they arise early disease, report on disease-associated perturbations cellular proteomes, static response other common stimuli, yet measurable the periphery, potentially well advance of onset clinical symptoms. Methods Here, we used high-throughput, custom cancer...

ABSTRACT Background The impact of healthcare disparities on the outcomes for hepatocellular carcinoma (HCC) has not been explored in South Africa. This study aims to evaluate and compare presentation, treatment, HCC a cohort patients treated public private sectors. Methods records 551 consecutive at hospital compared those 51 sector from 1 December 2001 29 February 2024 were retrospectively reviewed. Results Patients managed significantly younger (mean age: 49.6 ± 14.8 vs. 59.6 14.3 years, p...
Abstract Background Patients co-infected with hepatitis B virus (HBV) and the human immunodeficiency (HIV) are at risk of developing hepatocellular carcinoma (HCC). In sub-Saharan Africa, overlap between high HIV HBV prevalence may increase incidence HCC. This study investigated impact HBV/HIV co-infection on age presentation survival Methods Ethical approval was obtained to recruit, following informed written consent, patients diagnosed HCC oncology units four South African hospitals....
A bile leak is an infrequent but potentially serious complication after biliary tract surgery. Endoscopic intervention widely accepted as the treatment of choice. This study assessed effectiveness endoscopic retrograde cholangiopancreatography (ERCP), sphincterotomy and stenting in management postoperative leaks. An ERCP database a tertiary referral centre was reviewed retrospectively to identify all patients with leaks laparoscopic cholecystectomy. Patient records endoscopy reports were...
The aim of this study was to determine the safety and clinical effectiveness 10Fr plastic biliary stents compared uncovered self-expanding metal (SEMS) for palliative treatment patients with inoperable extra-hepatic malignant obstruction in a public hospital South Africa. From January 2009 December 2013, 40 who were admitted tertiary academic centre because distal enrolled prospective randomized study. Patients randomly assigned receive an SEMS or stent deployed through stricture during...
A left-sided gallbladder is a rare biliary anomaly with prevalence of 0.2%. Its identification important because the numerous and potentially hazardous associated vascular anomalies that may be encountered during surgery.
Introduction: Lymph-nodal involvement (N+) represents an adverse prognostic factor after pancreatoduodenectomy (PD) for pancreatic adenocarcinoma (PDAC). Preoperative diagnostic and staging modalities lack sensitivity identifying N+. This study aimed to investigate preoperative CA19.9 in predicting the N+ stage resectable-PDAC (R-PDAC). Methods: Patients included a multi-institutional retrospective database of PDs performed R-PDAC from January 2000 June 2021 were analyzed. A laboratory value...
Jaundice is a marker of advanced disease and poor outcomes in hepatocellular carcinoma (HCC). The aim this study was to describe analyse the management jaundiced HCC patients at large academic referral centre sub-Saharan Africa (SSA).
Treatment of major iatrogenic and non-iatrogenic bile duct injury (BDI) often requires delayed surgery with interim external biliary drainage. Percutaneous transhepatic cholangiography (PTC) catheter placement endoscopic retrograde (ERC) stent have been used to bridge defects. In some patients, bridging the defect cannot be achieved through ERC or PTC alone. Two patients BDIs, one underwent an extraluminal PTC/ERC rendezvous a fully covered self-expandable metal (SEMS) for acute management...
Defining the advantages and exposing limitations of endoscopic variceal ligation in controlling acute bleeding achieving complete eradication
The term portal biliopathy (PB) is used to describe the biliary abnormalities associated with hypertension. Between 5% and 30% of patients PB develop obstruction. We report on a patient extrahepatic obstruction caused by that was successfully managed an intrahepatic segment 3 bypass. traditional surgical approach for would be portosystemic shunt followed hepaticojejenostomy if jaundice persited. An bypass provides definitive treatment ensuring decompression stone removal in single procedure...
Variceal bleeding (VB) is the leading cause of death in cirrhotic patients with oesophageal varices. We evaluated efficacy emergency endoscopic intervention controlling acute variceal and preventing rebleeding during index hospital admission a large cohort consecutively treated alcoholic after first bleed. From January 1984 to August 2011, 448 (349 men, 99 women; median age 50 years) VB underwent treatments (556 emergency, 249 elective) admission. Endoscopic control initial bleeding, survival...
A young woman with persistent postprandial vomiting was found to have a high-grade proximal jejunal stricture. The stricture surgically excised, and histopathological examination showed gastric heterotopia localised ulceration fibrosis. Symptomatic in the small bowel is rare, our knowledge this first report of resulting subsequent stricturing obstruction.
